Chronic Pain and Medical Marijuana

Chronic pain can come from many causes, including arthritis, inflammation, back and neck problems, nerve pain like neuropathy and sciatica, migraine/headache disorders, fibromyalgia, as well as pain related to past injuries or surgeries. Cannabis products do not “cure” the underlying cause of chronic pain, but many people use cannabinoids as part of a broader plan to reduce symptoms, avoid unwanted side effects of standard pain medications, and improve daily function. The most commonly discussed cannabinoids for pain are THC, CBD, and more recently, CBG.

Reducing medication burden for some patients: In certain cases, symptom relief from cannabis may allow patients to use lower doses of other medications that cause side effects such as sedation or constipation. This is individual and depends on the overall care plan.

What THC often helps with

Pain intensity and pain-related distress: THC can change how the brain processes pain signals, which reduces the “sharpness” of pain or make it feel more manageable for many patients.

Nerve pain: Sometimes called neuropathic pain, or neuropathy, nerve pain is often described as burning, tingling, shooting, and/or electric-like sensations. Studies show that nerve pain is where cannabinoids have some of their strongest pain relief effects. THC-containing products are often reported as helpful along with, and sometimes replacing, more common, traditional pain treatments.

Sleep disturbances: When pain prevents restful sleep, THC, especially the indica strains,  help many people fall asleep more easily or stay asleep longer, which can indirectly improve daytime pain tolerance and wellbeing.

What CBD often helps with

Inflammation-related discomfort and general pain support: CBD is often used for inflammatory discomfort and overall pain support. Research is mixed depending on the pain condition, but some patients report meaningful benefits, especially for day-to-day aches, stiffness, and flare-ups.

Anxiety/stress that worsens pain: Chronic pain and stress feed into each other. Some people find CBD helps them feel calmer, which can reduce muscle tension and improve coping with pain.

A non-intoxicating option: CBD is not typically intoxicating, which is one reason some people prefer it for daytime use. That said, it can still cause side effects in some individuals and may interact with certain medications.

What CBG often helps with

CBG (cannabigerol) is a cannabinoid that is gaining interest for wellness and symptom support. Some patients use CBG for:

General discomfort and body aches: Early research and patient reports suggest CBG may have potential in pain and inflammation pathways, though strong clinical conclusions aren’t available yet.

Nerve-related discomfort: CBG interacts with several receptor systems involved in pain signaling. Some people report it helps with “wired” or nerve-type discomfort, but responses vary.

Daytime function and focus: Many patients describe CBG as a more “clear-headed” option than straight THC, which is one reason CBG is sometimes used during the day.

Cannabis also has the potential for interactions with other meds, most notably sleep meds, anxiety meds, pain meds, and anti‑seizure meds. Check with your physician for interactions, and report any changes.

Important considerations

Possible downsides of including THC include memory and concentration problems, slower reaction time (important for driving/work safety), dizziness or balance issues, worsened motivation or “brain fog”, and dependence risk with frequent/high THC use.

Bottom line

For chronic pain, cannabis products can help some people reduce pain intensity, improve sleep, and support overall joy and quality of life. THC is more often linked with stronger pain relief but more impairment risk, while CBD is commonly used for non‑intoxicating support and inflammation-related discomfort. CBG is also sometimes used for daytime comfort and general pain relief. Combination products are also an option, and often prove to be the most effective.
